We provide home health aide, live-in care, companion care, and related health and personal care services to clients in the Chicago metropolitan area.

All clinical services are provided pursuant to a physician's plan of care and applicable Illinois and federal licensing requirements. The scope, frequency, and type of services provided to each client will be set forth in a written Service Agreement and individualized care plan, which constitute the binding terms of the services to be delivered.

To enable the safe and effective delivery of care, clients and their authorized representatives agree to:
We accept payment from Medicare, Illinois Medicaid, most major private insurance carriers, and private-pay arrangements. Applicable rates, billing cycles, and covered services will be set forth in your Service Agreement.
You are responsible for any balances not covered by insurance or government programs, including applicable co-payments, deductibles, and services not covered under your plan. We will provide advance notice of any costs that are your responsibility, to the extent reasonably known.
If you believe a charge on your bill is inaccurate, please notify our billing department in writing within 30 days of receiving the statement. We will investigate and respond within 30 days of receiving your written dispute.
Accounts that remain unpaid beyond the terms specified in the Service Agreement may be subject to collection proceedings. We will make reasonable efforts to contact you before initiating any collection action. Pursuit of unpaid balances does not constitute a waiver of any other rights we may have under the Service Agreement or applicable law.
As a courtesy, we will submit claims to your primary insurer or government payer on your behalf where applicable. You are responsible for ensuring that we have accurate and current insurance information at all times. We are not responsible for denials arising from inaccurate or outdated information provided by you.

These Terms of Service, and any disputes arising out of or related to them or our services, shall be governed by and construed in accordance with the laws of the State of Illinois, without regard to its conflict of law provisions. Any legal action or proceeding arising under these Terms shall be brought exclusively in the state or federal courts located in Cook County, Illinois, and you hereby consent to the personal jurisdiction of those courts.

As a licensed home health agency in Illinois, we maintain a formal grievance procedure. Clients have the right to file a formal complaint regarding the quality of care or services received. Written complaints will be acknowledged within 5 business days and fully investigated within 30 days. You may also file a complaint with the Illinois Department of Public Health at 1-800-252-4343.
If any provision of these Terms is found to be invalid, illegal, or unenforceable by a court of competent jurisdiction, the remaining provisions shall continue in full force and effect.
